What to Bring to your Appointment
Registration Application - 15 and younger
Long form birth certificate - with both parent’s name(s) (Required)
If there are legal proceeding for the child, please bring paperwork.
ID / Drivers License (Parent) (Required & must be valid)
2 identical, unaltered passport style photos (of Child)
Only parents on Child’s Birth certificate @ appointment
Registration Application – 16 and older
Long form birth certificate – with both parent’s name(s) (Required)
ID / Drivers License (Required & must be valid)
2 identical, unaltered passport style photos
Please bring all documentation pertaining to Indian ancestry
SCIS Application - 15 and younger
Status Card (for Child) or 10-digit band number
ID / Drivers License (Parent) (Required & must be valid)
Status card / Full 10-digit band number (Parent)
SCIS Application - 16 and older
ID / Drivers License (Required & must be valid)

Guarantor
The Guarantor Must:
Be 18 or older
Have known you for at least 2 years
Be available and capable of answering questions about you if contacted by Indigenous Services Canada. (e.g. your name, approximate age, place of birth, physical description and place of residence)
be registered under the Indian Act
